New patients are welcome. A first visit is for listening, an examination, and a plain explanation of your options. The office confirms the time after you call or send a request.

Payment methods on file

The practice lists Visa, Mastercard, Discover, and CareCredit. This website does not take card payments yet. No fee is collected when you request a visit.

Insurance names on file

The current site lists Delta Dental, Dental Network of America, MetLife, BCBS, Cigna, and other PPO plans. Benefits are not verified on this website. Ask the office to confirm your plan before treatment.

Patient forms

The existing website names three forms: Account Information, Health History, and Consent for Use. Working copies of those files were not available during the rebuild, so they are not linked here. Call the office and the team can tell you how to complete them.

What still needs the office

Online scheduling is not connected to Google Calendar. The form cannot see open chairs or stop two people from choosing the same time. Online payment is not active, because the practice has not yet chosen a deposit, a booking fee, or a merchant account.
